Regardless of the technique used in cleaning and shaping, procedural errors can occur.  These included loss of working length, apical transportation, apical perforation, lateral stripping and instrument fracture. 

Loss of working length has several causes.  These include failure to have an adequate reference point from which the corrected working length is determined, packing tissue and debris in the apical portion of the canal, ledge formation, and inaccurate measurements.

 Apical transportation and zipping occurs when the force of the file exceeds the threshold for cutting dentin in cylindrical non-tapering curved canal . When this apical transportation continues with larger and larger files, a “teardrop” shape develops and perforation can occur apically on the lateral root surface .  Transportation in curved canals begins with a size #25 file.    Enlargement of curved canals at the corrected working length beyond a size #25 file should be done only when an adequate coronal flare is developed.

        mostly used for cleaning and shaping the root canal system.
        They are firstly precision ground in such a way as to have a quadrangular cross-section and then twisted clock wise to achieve the definitive form.
        The number of spirals per mm (pitch) for stainless steel files can very slightly depend on the manufacturers but is always more (generally double) than that of the reamers; their blades are furthermore positioned perpendicular to the long axis of the instrument giving files a particularly efficient cutting action during filing.

NEGOTIATING TO THE APEX in order to gain  working length is not always an easy task.  In some instances, the canals are blocked with calcium, but most of the time the canal makes a sharp turn that makes it impossible for a straight instrument to get around or past that bend in the root. 

The answer is always the same: pre-bend the endodontic instrument (8 0r 10 size k file). A bent instrument will never get you into trouble. 

A straight instrument in a curved canal can lead to several iatrogenic problems.

ONE OF THE largest causes of endodontic failure is infection. Infection can be the reason for the tooth’s needing endodontics in the first place, or it can be the cause of retreating an already endodontically treated tooth. The causes of infection include caries, fracture, excessive trauma from operative or prosthetic preparation, leakage of restorations, missed canals or anatomy when doing the endodontics initially, inadequate instrumentation of the canal, and calcified canals. Endodontic infections are generally caused by multiple species of bacteria living on the canal walls or in the dentinal tubules.Many times, the bacteria can be found in biofilms attached to the walls and entering into the tubules. The biofilm coats the bacteria in a film that makes it much harder to kill the bacteria—in other words, much more difficult to get rid of the infection.
